Northside OB/GYN is open to new OB and GYN patients. It is our goal to see each patient as close to the scheduled appointment time as possible. However, the practice requires each patient to have a chart with information needed for evaluation of your medical condition or to verify your insurance coverage. This is why we ask patients to arrive 10-15 minutes early to complete a number of forms at the first visit or to update the information at subsequent visits. Often there is a delay if the clinical or other file information is missing.
To minimize the possibility of such a delay, you can help by having these patient forms completed before arriving for your visit. Many of our patients also find it easier to complete the medical history forms at home where they may have access to information not brought to the office visit.
